Complete Daily Prior to School/Work*
- Has your child had a COVID-19 test and are awaiting results? If yes, then stay home.
- Have you (Parent/Guardian) had a COVID-19 test and are awaiting results? If yes, consult school nurse before sending your child to school.
- Take your (staff member) /or your student's temperature. If 100.4 degrees Fahrenheit or higher, then stay home.
- Are you (staff member)/is the student taking any medication to treat or reduce a fever such as Ibuprofen (i.e. Advil, Motrin) or Acetaminophen (Tylenol)? If yes, then stay home.
- Are you (staff member) /is the student experiencing any of the following?
Stay home and consult your physician if you (staff member) or your child:
• Have one or more symptoms in Group A OR
• Have two or more symptoms in Group B OR
• Are taking fever reducing medication
• Have one or more symptoms in Group A OR
• Have two or more symptoms in Group B OR
• Are taking fever reducing medication
